hibernate save update problem

Hi,
I am workin in a application (web application built with struts - spring - hibernate -Oracle database). normaly it have over 40 visitors. when some concurrent users (<10) access the same program(ex; updating different instants of same entity class or updating different rows in same table). Some users fail to do the operation and some are succeeded.And there was no error in the logger.but operation fails. Can some one guess what is happening there ?.

How do you know they failed?
