I see this quite frequently where my total number of hits is larger than the actual number of objects returned by Hibernate Search. I know the docs say this is approximate, but they example they give are asynchronous clustering which I'm not using. I'm using pretty standard out of the box operation. Why would I see this discrepency? How can I debug it or see why there's a problem? I've tried to reindex my DB in order to correct the problem, but that doesn't seem to help.

Which version are you using? Are you using setMaxResult?
Can you paste the code creating and executing the query?
